Practice and Placement: Report a Concern

In most circumstances, anonymous reporting makes it difficult for the University to follow up and address concerns either formally or informally. An anonymous report will still be reviewed but the University may not be able to take any further action.

Alternatively please consider submitting a report as a member of public or as a student / member of staff.

Who should complete this form?

Anyone involved in the teaching and supervision of University of Worcester Health, Care or Social Work student, those, including other students, who have concerns about the wellbeing and behaviour of students in Health courses This may include patients and other healthcare service users who have contact with health students; anyone who observes the behaviour of a health student which they consider could be detrimental to the student, staff, current or future patients.

When should this form be used?

Only rarely will student behaviour give rise to an immediate patient safety issue; nevertheless, when it does action should be taken quickly. Occasionally student behaviour will give rise to a Fitness to Practise issue. However the great majority of concerns relate to a student’s wellbeing.

I am not sure if the behaviour I have observed is really something that the University or the Professional Body/Regulator would be interested in.

If you are worried about a student, it is better to let us know. Other people might also have low level concern about a student which taken together paint a broader picture. If we do not find any cause for concern, the student’s progress and training will not be affected, If we do, we might be able to help and support the student. We would encourage you to let the student know that you are submitting the form so that they understand your intention is to help them rather than punish them.

What will happen next?

The University takes all such concerns very seriously and will investigate, following our established process. The student will be told of the concern and contacted. You might also be contacted for further information. The University has a range of possible outcomes depending on the severity of the concerns, but might include supplying additional pastoral or academic help for a student.

What will happen to me if I complete one of these forms?

The process of investigating a concern is open and transparent. We want to encourage anyone who has concerns to report them to us in confidence. If you concern triggers a formal investigation, we will feedback the outcome to you. If you are not satisfied that we have adequately dealt with your concerns you may contact the appropriate Head of School.

Who should complete this form?

If you are concerned about patient safety or any element of your clinical placement, it is better to let us know. Other people might also have low level concerns, which taken together, paint a broader picture. We would encourage you to discuss your concerns with your Placement Supervisor/Placement Lead and let know that you are submitting the form.

You may also be contacted for further information. The University has a range of possible outcomes depending on the severity of the concerns, which may include action plans or removing students from placements in the organisation.
